Self-management gives you the power to source your own support workers and negotiate rates you’re comfortable paying. You have the flexibility to work with care services that meet your lifestyle needs.
Trilogy Care’s team supports you by developing your care plan, including establishing your care goals and focusing on the financial side of your package, such as paying invoices, reimbursements, and compliance checking your workers.
Self-management is ideal for people who want to feel empowered while managing their Home Care Package.
Our services
Trilogy Care is a registered Home Care Package provider. We offer two levels of self-managed home care – Self-Management and Self-Management Plus.
The Self-Management option allows you to take full control over your Home Care Package. The Self-Management Plus option is for those who need some care coordination support.
Self-Management
Our Self-Management option is for those who want to retain full control over your care services. The service includes:
• Dedicated Care Partner who is available for you to contact directly via the phone
• Your personalised care plan, with regular reviews according to your care needs
• Access to a team of clinical nurses to guide you on your care journey
• Compliance checking of your care providers
• Budget management
• 15% of your Home Care Package with no hidden fees or surcharges.
Self-Management Plus
Our Self-Management Plus option is designed for those who would like to have support with care coordination. Self-Management Plus includes:
• Dedicated Care Partner who is available for you to contact directly via the phone
• Your personalised care plan, with regular reviews according to your care needs
• Access to a team of clinical nurses to guide you on your care journey
• Compliance checking of your care providers
• Budget management, and
• Care coordination
• 26% of your Home Care Package with no hidden fees or surcharges
Pricing
Both our Self-Managed and Self-Managed Plus service options are charged at a flat percentage fee of your approved Home Care Package budget. There are no daily management fees, hidden costs, or surcharges! Our transparent pricing model means you can be reassured your Home Care Package budget will be maximised for the care and support you need.
