Managing your services


There may come a time when your needs change or your support services no longer suit you.


Perhaps you would like to change providers, adjust your services or require a higher level of support. For example, you were originally allocated a Home Care Package Level 2 to support you, but after two years your mobility has significantly decreased and you need more help at home than what is provided under this Package. 


Or maybe you would like to reallocate funds from meal planning services to transport services and get some help with personal care. 


Personal goals are an important part of your care plan and those may change over time. Goals such as maintaining a healthy lifestyle, improving mobility or achieving independence can guide your choice of care and services and should also be taken into consideration during a review.
