Over time, it is likely that your health and personal care needs will change and any aged care services you receive should be adaptable to those changes.

It is important you know who you can talk to when you need to change services or update your care plan and how the process works.

Talk to your provider if you need to make changes to any of the services you receive as well as the process of organising a reassessment if you need it.
