Our team of specialist Dementia Care Consultants provide ongoing support and education to people living with all forms and stages of dementia, including Alzheimer’s Disease, their families and carers & to other Southern Cross Care (WA) Inc staff. Our services are available in the community or in one of our aged care homes.
In developing a comprehensive individualised dementia specific care plan, we can assist with ongoing assessment and care planning to ensure there is the right support for an individual’s needs, choices and aspirations.
For people living with dementia who wish to remain at home, we offer three specific respite centres in the Perth Metropolitan area providing valuable support to carers. These have been specifically designed as an alternative to traditional day centres by creating a ‘home away from home’ which is warm, comfortable and welcoming. Our services can be used as a once off service or on a regular basis. We also offer carer support sessions to help people in similar situations connect & learn about practical tools & resources to help carers enjoy a quality & balanced life.
Dementia is a broad term that describes the loss of memory, social skills, intellect and usual emotional reactions. SCC Dementia Services understands the impact and challenges a person diagnosed with dementia and their family members experience during their everyday lives.
The central focus for the support provided through our services is to enable understanding, empathy and awareness of the challenges faced by their loved ones who have been diagnosed with dementia.
