Home and centre based nursing is available to frail aged and younger disabled members of the community. Assistance is also available for people requiring post acute care.
South Eastern Community Care (SECC) is a not for profit organisation which has operated for over 40 years. Services are provided to frail aged and younger disabled members of the community including special needs groups from Non English speaking backgrounds, Aboriginal and Torres Strait Islander descent, rural and remote areas and financially disadvantaged clients.
Access to all services and programs is by referral from a Medical Practitioner, the Aged Care Assessment Team, hospitals, health professionals, or by a self-referral. Eligibility for services is then determined through an assessment process.
Holistic quality care is provided by an experienced and qualified team of professional staff. Established policies and procedures are in place to meet service standards.
SECC manage personal information in compliance with the Privacy Act and are committed to protecting and maintaining privacy, accuracy and security of personal information.
SECC is funded by the Australian Government Department of Social Services, the State Department of Health & Human Services, Home and Community Care Program and community donations.
