We are private facility located in Guildford. We are located next to two parks which provide our clients a platform to go for walks and perform exercises. We are a small secure facility and dedicate our time and commitment to your loved one. We are a dementia specialist facility and our team are trained to deliver this services. We have visiting in house General Medical Officers that attends to all medical treatments alongside our Clinical Coordinator. Our full time OT supports our clients to meet their daily activity goals.
Having a small team here at Guildford, you get to discuss any matters directly with us and addressed immediately. Our home provides a home like environment and we strongly encourage families to bring items that create their room to feel homely. All our bedroom is single and with their private enuite. The rooms are equipped with remote control bed, lockable bedside table, reserve cycle aircon and an emergency bell for clients to use.
We do request for the following documentation:
• ACAR ( aged care assessment report)
• Assets Assessment ( if available)
• Power of Attorney or Guardianship orders
