Elderly most impacted by medication errors
The National Prescribing Service has published a review of medication errors outside hospitals. It says that the elderly, the very ill and those being passed from one part of the health system to another, are most at risk.
The most commonly reported contributing factor of medication errors and adverse events was poor communication, which is highlighted when patients are transferred between hospital and community settings.
