Diversicare Home Care Packages Diversicare’s Home Care packages (HCP) are delivered through Consumer Directed Care (CDC) services which allow consumers to have greater control over their own lives by allowing them to make choices about the types of aged care and services they access. Under the CDC approach, consumers are encouraged to identify goals which can include independence, wellness & re-ablement. The consumer decides the level of involvement they wish to have in managing their package, which could range from involvement in all aspects of the package, including coordination of care and services, to a less active role in decision making & management of the package. There is ongoing monitoring and a formal re-assessment (at least every 12 months) to ensure that the package continues to be appropriate for the consumer. Each service is individually designed to assist consumers with complex care needs to continue to remain living in their own homes and in the wider community. These services are provided by Diversicare’s trained bilingual home care workers. Our services take into account culture, language and the needs of the consumer and their carer. Through the introduction of an individualised budget, CDC provides greater transparency to the consumer about what funding is available under the package and how those funds are spent. Brokerage Services for the various language groups and Private Fee for Service provision is also available.
