Home Support Programs
Liberty Community Connect provides a range of in home and community services that support people living with a disability or older people to live meaningful, active and socially connected lives. Our in home services are about supporting people to live independently in the community.

Social Support
Liberty’s social support includes shopping, banking, bill paying, companionship and outings, appointments, pharmacy needs, letter writing and doing whatever is needed to support you at home and in the community.

Personal Care
Liberty’s personal care involves assistance in daily needs such as personal hygiene and grooming, medication, and mobility assistance.

Domestic Assistance
Liberty’s domestic assistance involves assistance in light household duties including house work, spring cleaning, laundry, ironing, bed linen changing, and meal preparation. We Come along aside to work with you, to enable you to do to the things you can.

Respite
Liberty’s respite care is about giving carers regular time out to pursue hobbies, interests and to take a break.
