South West Community Transport receives funding under the Commonwealth Home Support Programme (CHSP) and is a registered NDIS provider, supporting older people and people with disability to live independently, stay connected, and access their community with confidence.South West Community Transport provides transport services to individuals 65 years and over (50 and over for Aboriginal and Torres Strait Islanders), registered NDIS participants, carers and those who may be transport disadvantaged.We operate a dedicated fleet of accessible vehicles across the Fairfield, Liverpool, Campbelltown, Camden, Wollondilly, and Wingecarribee Local Government Areas, delivering safe, reliable, and affordable door-to-door transport.If you are an NDIS participant living outside these areas, we can still assist through our partnerships with local taxi providers nationwide, ensuring you have access to safe and dependable transport anywhere in Australia.Our transport services help clients remain independent and socially connected by providing access to:Medical and doctors’ appointmentsShopping trips and errandsExercise and wellness classesSocial outings and community eventsVisiting family and friendsClubs, community groups, and church activitiesAccess to work, school and your communityIn addition to transport, we also provide all three levels of NDIS Support Coordination, assisting participants to connect with the right supports and make the most of their plans:Support Connection (Level 1) – Build your ability to connect with informal, community, and funded supports.Support Coordination (Level 2) – We work with you to strengthen your capacity to live independently, maintain relationships, and access your community.Specialised Support Coordination (Level 3) – Higher-level support from qualified specialists to manage complex situations and ensure consistent delivery of services.At South West Community Transport, we proudly support NDIS participants and seniors across South West Sydney with our local fleet, and through our nationwide partnerships, ensuring everyone can stay moving, connected, and independent.
