MannaCare Community Care

MannaCare Community Care provides advice and tailored support to people on Levels 1 to 4 Home Care Packages living in the municipality of Manningham and surrounds.

These packages are aimed at older people and are designed to enable them to remain living independently in their community. The packages are funded by the Commonwealth Department of Health. The subsidy covers the costs of administering the program, providing advice and support and purchasing of eligible goods and services on behalf of each client.

Clients, in conjunction with their Care Advisor, identify goods and services they wish to purchase to assist them to maintain their wellbeing. Clients may also self manage services if they choose to. Home Care Packages includes a core component of case management, and other services can include additional case management, personal care, nursing, oxygen and enteral feeding, counselling, social support, transport to appointments, home help, meal preparation, garden maintenance and wellbeing services. Clients will receive a monthly statement showing income, expenditure of services and any unspent funds.

The MannaCare team can work with people and family members to establish a package that suits an older person’s needs.

To receive a package, clients must first be referred for an assessment by an Aged Care Assessment Service as requiring packaged care. If eligible, their name can then be placed on a national register for the appropriate package. Several organisations manage packages in the municipality of Manningham. Government mandated fees apply for users of Home Care Packages.

MannaCare also offers an in-home support service, MCA FlexiCare, which can provide a range of respite & home care services. More broadly, MannaCare offers an integrated range of local services designed to assist people to access services to enable them to remain living in their community.
