Understanding Assessment and AccessAn assessment is required from My Aged Care to access any of our services, In-Home or at our Day Centre, at a reduced cost. You can contact My Aged Care on 1800 200 422 to arrange an assessment and request a referral to Astley Care for support. If you need help with your assessment, please let us know. Our CHSP In-Home Services are tailored to your needs:Domestic AssistanceWe can support you with basic housekeeping tasks to keep you as independent as possible in your home. Helping with the things you find more difficult, such as vacuuming, washing floors, laundry, and meal preparation, ensures you can keep doing the things that are important to you. Shopping and TransportIf you are having difficulty accessing your community, the shopping centre and medical appointments, Astley Care can transport you when you need help. For many of us, shopping is not just a task. Shopping is an essential social activity that helps them keep in touch with the broader community. We offer support for independent shopping or transport, where your driver or support staff member accompanies you to your destination. At Astley Care, we also provide social shopping buses; this group of people go together and have a social morning tea whilst completing their shopping. Transport is adaptive support to access medical care, attend appointments in the community, visit family or friends, and shop. We want to help you access places that are important to you. Astley Care has an array of vehicles, including modified hoist-equipped vehicles; we ensure that your needs are supported by the right vehicle for you.Social Support One to OneDo you wish you could go into the community? Being limited in your social interaction or being socially isolated can significantly impact your health and well-being. Astley Care values physical and mental well-being; we encourage our clients to participate in community activities if they want to. Astley Care provides a wide range of social support outings, including group and one-to-one visits. Our Support Workers can visit you at home and take you out to somewhere of your choice. Sometimes you may not feel like going out; we can also arrange for one of our staff to visit your home for a social visit or for someone to telephone you regularly to chat. We support you in exploring all the options in your support requirements. Personal CareLooking after your personal care needs is a sensitive topic. Our staff can come to your home to help you maintain your dignity and privacy by providing assistance with tasks you may find a little difficult. Some of the tasks we offer, if needed, are helping you with your showering, bathing, dressing, and shaving, as well as medication prompting and management. Home Maintenance/GardeningOur home maintenance service, which includes gardening and other light maintenance tasks, is provided to enable you to live safely in your home. Services include lawn mowing, pruning of shrubs, cleaning gutters, changing light bulbs and smoke alarm batteries and other odd jobs.Respite & Carer Support – in the HomeTaking on a carer role is a role we don't take lightly. It can be one of the most rewarding roles, whilst also being exhausting at times. Sometimes, a carer may want to do something special for themselves; if you don't feel you can do this in your caring role or don't do this often enough, we can help by supporting your loved one while you go out. Our support workers are well trained, including in dementia care, and will provide company in the home, outings, personal care, meal preparation and light housework to give the carer a break. Respite can be provided on a regular, planned, or one-off basis and is available on weekdays. Limited hours are also open on weekends and evenings to enable carers to attend special occasions. Please know you don't have to be an 'official' registered carer for access to this service.
