Family Based Care is a independent Tasmanian community based "not for profit" organisation that has been providing quality individualised support services for people living with a disability, the frail aged and their carers for over thirty years in Tasmania.

Our support services are delivered in a manner which respects and responds to our clients changing needs. Client choice and control are central to our reablement and wellness approach. Our motto is "Your Care, Your Way".

With offices in Burnie, Devonport, Launceston, Rosebery and Hobart our clients and their family can meet face to face with our experienced team of coordinators to discuss their support needs and ongoing care.

Family Based Care is an "Employer of Choice" investing in the development and training of all our employees to ensure we can deliver consistent, quality, high value support to the communities of Tasmania.

Family Based Care adds value to care by providing additional services, like service navigation, social groups and other programs funded by the association in support of the communities we work in.
