We’re guides. We make things happen. We don’t provide care directly but, as our name suggests, we’ll work with you to work out what your needs are, and then connect you with the care you require. We specialise in ageing, disability, mental health and carer support.
All your needs are connected, so we look at everything, from medical treatment to help with the housework. In order to keep living independently it all needs to be taken care of.

We’re a non religious, not-for-profit organisation, and we deal with hundreds of highly qualified care providers. We are fully independent & our commitment is always first & foremost to you.

Access and Support Program: links for people from the West Melbourne Aboriginal Community to services and supports.
Adult Flexible Respite Program: provides respite options including individual brokered respite, respite social groups and one-off emergency respite funding.
Daylesford Retreat: offers short term holiday respite for carers (and their families).
Home Care Levels 1 & 2: this package offers support to older adults with low level care needs who are living at home.
Home Care Levels 3 & 4: this package offers support to older adults with high level care needs who are living at home.
Homeshare: A simple and effective way of meeting people's support and housing needs. Householders are matched with a homesharer who provides practical support in exchange for accommodation.
Linkages Program: assists people with complex care needs to live independently by providing individually tailored packages of care. It aims to prevent premature or inappropriate admission to residential care.
Planned Activity Group: a safe and positive social environment for members of the Aboriginal Community.
Respite for Older Carers: short term assistance and support to older carers who care for a person with a disability.
Veterans’ Home Care Assessment and Coordination: low level home care services to eligible veterans, war widows and widowers.
Care Connect requests a client contribution to the cost of services in accordance with Commonwealth Government guidelines.
