Uniting AgeWell Hume Home Care supports seniors in the Hume region to continue to live well at home and remain healthy, active and socially connected in their community. 

We provide support at home services in the LGAs of Alpine, Benalla, Indigo, Mansfield, Mitchell, Moira, Greater Shepparton, Wangaratta and Wodonga.

We take your My Aged Care Support Plan and transform it into personalised, coordinated care that keeps you safe, well and independent at home.

Through careful planning, regular monitoring and strong partnerships with your health team, we’ll ensure you receive maximum benefit from your approved Support at Home services and funding.

Your Uniting AgeWell Care Advisor will work with you to navigate the Support at Home program, optimise your budget and adapt services as your needs change. Together, we will create a comprehensive support system to help you thrive at home.

The Support at Home program will provide you with access to an approved list of services.

Our dedicated team will support you to choose or change the mix of services you can access within your budget and create a Support Plan that best meets your needs.

Your individual Support Plan allows access to three service categories - Clinical Care, Independence and Everyday Living. 

Support at Home services may include:

Clinical Care

• Nursing services
• Allied health (physiotherapy, occupational therapy, podiatry, etc.)
• Nutrition support
• Clinical care management

Independence

• Personal care
• Social support
• Respite care
• Transport
• Assistive technology

Everyday Living

• Domestic assistance
• Home maintenance
• Meal preparation and delivery
• Garden maintenance

Under the program, there are eight ongoing service classification levels and corresponding government-subsidised funding levels (budgets) available.

You will be allocated a Support at Home classification from My Aged Care based on your aged care assessment and eligibility.
