Commonwealth Home Support Program
The Commonwealth Home Support Programme (CHSP) helps older Australians access entry-level support services to live independently and safely at home. If you need a small amount of help with everyday tasks, or occasional assistance, Life Care's CHSP services could be right for you. CHSP is about providing timely, low-intensity support to help you maintain your independence and stay connected with your community.
Life Care offers a range of subsidised CHSP services, including:
Domestic Assistance: Help with tasks like cleaning, laundry, and light shopping.
Transport: Assistance to run errands or get to appointments and social activities.
Personal Care: Help with self-care tasks like dressing and grooming.
Nursing: Basic nursing care provided in your home.
Gardening & Home Maintenance: Minor Garden upkeep and home repairs.
Social Support – Individual & Group: Connecting with others, companionship, and group activities.
Allied Health: Services such as Physiotherapy and Occupational Therapy to maintain movement and function.
Home Care Packages
Home Care Packages are designed to give you choice and flexibility in the way that your care and services are provided. Life Care works with you to create a care plan tailored to your needs and preferences, within your package budget.
HCPs provide government-subsidised care services for older Australians with moderate to high support needs, enabling you to live independently in your own home for as long as possible. Life Care offers a wide array of services under your Home Care Package, tailored to your individual care plan. These can include:

There are four levels of Home Care Packages, starting at the Level 1 package which supports people with lower levels of needs though to a Level 4 package which supports people with complex care needs.
One of our Case Managers will meet with you to identify what your care needs and goals are and create a personalised care plan.
