The Government uses the term ‘reablement’ to refer to short term or time limited support that helps a person to adapt to changed circumstances, such as functional loss after an illness or accident, or to regain confidence and capacity to return to their previous level of activity. Resthaven’s Restorative Care Pathway is a time-limited, goal-oriented service that supports older people to recover after illness or injury, or to address early signs of functional decline. Over a 6-week period, the program helps you improve your ability to manage daily tasks, regain strength, and continue living independently. The Restorative Care Pathway is funded by the Support at Home program The focus of the multidisciplinary team approach is healthy, active ageing to assist quality of life and avoid unnecessary hospitalisation, and possible premature entry to residential aged care. Our team will work closely with you to create a personalised care plan that reflects your goals, lifestyle, and choices.
