Co.As.It. Community Services - QCSS

Support is available via the Queensland Community Support Scheme (QCSS) to people under 65 years of age who have a long-term disability, chronic illness, mental health, and other conditions that impact their day-to-day functional capacity and ability to participate in the community.

This program cannot support people who are NDIS eligible. Individuals will be required to undertake an assessment and register with the Statewide QCSS Access Point, which will provide a referral to the best-suited provider for that individual.

There are two support levels under this program:

• In-home supports

• Community connection supports
