Support is available to people under 65 years of age, who have a long-term disability, chronic illness, mental health, and other conditions that impact their day-to-day functional capacity and ability to participate in the community via the Queensland Community Support Scheme (QCSS).
This program cannot support people that are NDIS eligible. Individuals will be required to undertake an assessment and register with the Statewide QCSS Access Point who will provide a referral to the best-suited provider for that individual.
There are two support levels under this program:
• In-home supports
• Community connection supports
Support services have been approved by the State Government to be delivered under a Consortium model. The consortium consists of GOC Care, IAWAA Inc., and Co.As.It. Contact our QCSS Coordinator to discuss eligibility for these services on 07 3624 6100.
