About Us
CCH Home Care Services are focused on healthy ageing at home, with services tailored to meet your individual needs. This enables you to stay physically active, socially engaged, independent, and safe in the comfort of your own home.
As an approved provider under the Commonwealth Government’s Home Care Packages program, we offer a range of services through the My Aged Care scheme. If you need assistance with the My Aged Care registration process, we’re here to help.
If you aren’t eligible for a government-funded Home Care Package or are awaiting approval, our Home Care Services are also available on a fee-for-service basis.
Our Services
• Personal Care: Assistance with showering, dressing, mobility, toileting and more.
• Daily Living Support: Help with meal preparation, equipment to support independence, shopping and transportation.
• Health Management: Includes nursing services, allied health therapy, continence, and skin integrity management.
• Social and Emotional Support: Respite care, social activities, emotional support and cognitive care.
• Home Maintenance: Gardening, safety modifications, cleaning.
How It Works
• Customised Care Plans: We work with our clients to create a care plan addressing their goals, needs and preferences, including services from approved third-party providers (separate to CCH) if required.
• Flexible Care Levels: We cater to all Home Care Package Levels (Levels 1 – 4), as determined by the individual’s ACAT assessment.
• Care Management: Ongoing monitoring and adjustments to ensure the care plan continues to meet the client’s needs, goals and preferences.
