Who Are We? 

We are a Team of Clinicians –  highly skilled, experienced, caring and compassion Registered Nurses providing consumer directed (patient centered) care and services to Clients, Families and Service Providers.

Our team support with the following services :-

• Aged Care Placement

• RN Clinical assessments and Service Provision.

• MAC navigation and referrals to community services providers

• NDIS navigation and support.

• RN technical nursing service including continence care and management, wound care and management, medication administration and management and diabetes care and management. 

What is Care Consultancy & Choice

• Developed for our over 65 yo population and people under 65 yo with disabilities.

• Providing independent suggestions/ advice on all care options from care at home to 24/7 care for all age groups.

• Ensuring that consumers/families can make informed choices/decisions about their care options.

• Helping consumers/loved ones navigate, what can sometimes can be a time consuming and confusing system.

• First visit is complimentary to determine the care needs and service provision required.

Our Team accepts referrals for the Sunshine Coast, Greater Brisbane, Toowoomba and Gold Coast Regions.
