* Nursing services are only available through a Home Care Package. 

How to access Home Care 

If you're aged 65 and over, you may be eligible for government subsidy through the Commonwealth Home Support Program or a Home Care Package. You will require an assessment through My Aged Care to determine your eligibility and level of funding. 

Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Program is an entry-level program designed to provide basic level care and support services. The Regional Assessment Service (RAS) will determine your eligibility and provide referral codes for your apporved services. You are expected to co-contribute to the cost of your services.

Home Care Package (HCP)

Home Care Pacakges are designed for when you eed more than entry-level support and offer a coordinated approach with access to a greater range of services, aids, and equipment. There are four levels of subsudy available, supporting basic to high-level care needs. The Aged Care Assessment Team (ACAT) will determine your elibility and assigned Home Care Package subsidy level.
