Living independently in our own homes is important to most of us. For people who may for a variety of reasons find this difficult without assistance the Town of Bassendean’s Seniors and Disability Services is here to help.
With over 40 years’ experience assisting older people and people with disability, we are provide quality supports to enable you to continue living independently in your home and out in the community.
We partner with you to help you remain connected to your existing circle of family and friends as well as offer the opportunity to make new connections. We encourage you to make decisions and choices and actively participate in your own support services. Together we make sure that you, our client, is always receiving appropriate services that meet your changing needs to maintain your independence.
Home and Community Care (HACC)
The Town of Bassendean Seniors & Disability Services is a provider of Home and Community Care (HACC). These services include personal care, domestic assistance, home & garden maintenance, transport, meals delivered to the home or in home meal assistance, one on one and group social support (including shopping), respite and centre based events and activities. The following specialist services are also available under HACC:
- Sustainable Gardens Project
The aim is to assist clients who due to medical conditions and physical limitations have become restricted or are prevented from participating in gardening. We do this by helping HACC eligible clients establish an easy care, low maintenance water wise garden they can enjoy maintaining.
- Dementia Specific Services
This program provides specialist support to people experiencing dementia. There is a day program at our day centre as well as respite support in the home.
- The All In Program
The All In Program is a collection of specialist services delivered in an inclusive & welcoming way. The following services promote friendship, independence, dignity and a sense of belonging and are open to people with specific needs relating to mental health, intellectual or physical disability, as well as specific cultural or linguistic needs.
- Individual small group activities based at our Centre in Ashfield.
- Small individualised weekly group outings.
- The Wings Groups' Friday night program for people with a disability.
Home Care Packages (HCP) Level 1, 2, 3 and 4
The Town of Bassendean Seniors & Disability Services is a provider of Home Care Packages delivered using a Consumer Directed Care approach. This means you are at the centre of your care, in charge of making decisions and choices on what you need and how that need is met. We just facilitate and support you with the service choices you make.
Home care packages are available to assist frail aged people who have complex care needs, to be eligible to receive a package, an Aged Care Assessment Team Member (ACAT) needs to assess you to determine the level of assistance you require. Contact us if you would like us to support you through this process.
We also offer all our services on a private basis for clients wishing to directly purchase services and other organisations through brokerage arrangements.
