NSW Guardianship Tribunal changes forms

The NSW Guardianship Tribunal has re-designed its two most frequently used application forms, the ‘Application for Appointment of a Guardian and/or Financial Manager’ and the ‘Application for Consent to Medical or Dental Treatment’.

The new forms will replace the old forms and are available to download from the Tribunal’s website www.gt.nsw.gov.au.

The new forms assist applicants to understand when and how to apply and clarify the responsibilities of an applicant. They assist the Tribunal to identify risk factors for people with disabilities and quickly assess the urgency and complexity of applications. The new design is user friendly, accessible and up-to-date.

The Tribunal will accept old versions of the forms but service providers are asked to dispose of old forms and use the new improved versions.

The Tribunal’s online application facility will remain in its current format initially and online applications will be accepted as usual.
