The Victorian Health Department has prepared a checklist advising Victorians on key questions to ask health providers if they have to travel for health care.
This checklist will help you to explain your needs to health care providers. It covers three key stages in a health care journey: Before you leave home, At the hospital and Going Home. It provides handy travel and support contacts to help prepare for each stage of your journey for health care.
