SummitCare HomeCare provides support for you in the comfort of your own home. We offer a full range of care and support services that will satisfy all your needs. A dedicated Support Manager will coordinate all your care and support services, they will ensure you are happy with your choices, care plan and goals as well as assisting you to manage your finances. The home care team are all qualified and passionate people who come from culturally diverse backgrounds. We will aim to match your personal requirements where possible. In the majority of instances, once a carer is assigned to you, they will visit you each time you receive your service.
Personal Care – feeling good about yourself is an important part of total wellbeing, we can assist with showering, bathing and dressing as well as grooming.
Bedtime – this can often be a difficult time of the day, our carers can assist with bathing and personal hygiene and ensure bedtime is relaxed.
Accessing Allied Health Services – access to physiotherapists, optometrists, occupational therapists and complementary therapists that can support your wellbeing at home.
Cleaning –t idying, vacuuming, dusting, mopping floors, washing clothes, ironing and making beds.
Shopping – shopping and unpacking the shopping, preparing meals and clearing away after.
Home Maintenance and Modification – gutter cleaning, home modifications, simple repair work, painting touch ups, and regular changing of smoke detector batteries.
Garden Care – mowing, weeding, top dressing, pruning and even glass cleaning.
Pet Care – dog walking, pet grooming and veterinary visits. Bird cages and fish tanks can be cleaned and kitty litter trays cleaned and replenished.
Relocation Services – downsize, pack, arrange a removalist and unpack at your new home.
Dementia Care – offers support to people with dementia in their own familiar surroundings. This also enables the primary supporter to have a break. Our team are educated about dementia and understand the stress and emotion this illness can cause.
Diabetes Support – support with your Diabetes Management Plan in conjunction with your General Practitioner. This care is under the direction of a registered nurse and is regularly monitored.
Care Following Hospitalisation – This care is under the direction of a registered nurse and is closely monitored. It is not rehabilitation, rather a support service to get you back on your feet again at home.
Final Friend – support for in home palliative care in conjunction with your local Community Palliative Care Team. This support is under the direction of a registered nurse and is closely monitored.
Transport, Mobility & Assistive Technology
Mobile Care – can be arranged for journeys to and from social engagements and/or medical and allied health appointments, hairdressing and other events.
Mobility Aid Provision – assistive devices such as wheelchairs, bedside commodes, bed and chair raisers, continence aids, walking sticks and frames, easy reachers, electric beds.
Assistive Technology – is very important and will ensure your independence and capacity to remain in your own home for as long as possible. Eg. call pendants, security access systems, falls monitors and sensor lighting.
