Dementia Services | Southern Plus
About
At Southern Plus we understand everyone experiences dementia differently. That’s why we focus on responding to individual needs and circumstances to provide the best opportunities for our residents to stay connected and empowered to live their best life.
We provide a range of services catering for different care needs including temporary respite care, in-home support and high-level care in residential aged care homes.
Our goal is to empower people living with dementia to feel connected and engaged in their life.
Some people living with dementia may need to move into residential aged care and others might be able to continue living at home with a family member or carer. Caring for a person living with dementia can be both rewarding and challenging, and sometimes carers need a break to look after their own health. They can do this by applying for Government-subsidised respite care at our community-based respite centres or in-home support.
Our staff consider and appreciate our residents as members of the community considering their personality, learning preferences, and previous life experiences. Through improving awareness, knowledge, skill and support for our residents and clients, we develop a truly personalised and holistic behaviour support plan that is based on best practice care principles outlined by Dementia Australia.
