Southern Aged Care Placement Services will:
Assist our client to navigate the aged care process, complete and submit all relevant documentation and actively assist to find suitable Aged Care placement to meet the needs of our clients.
Assist our clients to understand, complete and submit the required documentation. This includes Income & Assets forms, Aged Care application form, nominee authority, Advanced Care Directives, etc.
Assist and support clients living at home to access community support and set up home care services to meet the clients assessed care needs.
After we develop, organise, facilitate and assist the client to receive appropriate care that they need, we will conduct a follow up to ensure that both the client and family are happy with services.
