Santa Clara Home Care is a registered Support at Home provider in Sydney, specialising in hospital discharge support and transition care for older Australians. We arrange care within 24–48 hours of onboarding, coordinating nursing, personal support, transport, and overnight services so clients can leave hospital safely and return home with confidence. Our registered nurses lead all care plans from day one.
We also provide ongoing in-home support including personal care, meal preparation, domestic assistance, companionship, and allied health services, available 7 days a week, including evenings, weekends, and public holidays. Services are available across all Sydney regions.
