These home care professionals can help you understand your options for in home supports, from Government funded supports through the Home Care Packages (HCP) program, to self funded support options.

Our experts are connected with a network of hundreds of accredited home care providers across the country, from small to large service providers.

When you call us on 07 3063 3769 or request a call back through the form above, one of our aged care coordinators will talk with you to understand your needs and circumstances.

They may then organise a face-to-face meeting, video or phone call with you and your family about your preferences and wishes to help them connect you with the providers that offer the best care and support for your needs and circumstances.

The consultant can also help you organise any assessments that are required to help with accessing Government funding, assist with forms and applications and answer any questions you may have about fees and charges.

They can assist you at any point during the process, whether you're just starting to look at options or are ready to find a provider. They'll guide you through the necessary assessments, advise you on the best home care options for you, help you understand fees and agreements and take care of all the paperwork.

With their expert advice, you can feel confident that you'll be able to make an informed choice when deciding on a service provider, and be supported by an independent advocate to answer any questions you may have along the way.

They are fully qualified case managers who will take the time to listen to you, understand your situation, answer any questions you may have and provide you with transparent and comprehensive advice. 

Consultants are based in the State or Territory they focus on and this local knowledge and expertise is important so that you can be confident they know the local area and providers.

How is this service free?

The Home Care Assistance Service is able to provide their personalised services free of charge to you because they are funded by the home care providers from their network.
