They may then organise a face-to-face meeting, video or phone call with you and your family about your preferences and wishes to help them connect you with the providers that offer the best care and support for your needs and circumstances.
They can also help you organise any assessments that are required to help with accessing Government funding, assist with forms and applications and answer any questions you may have about fees and charges.

The Aged Care Assistance Service is able to provide their personalised services free of charge to you because they are funded by the aged care providers from their network.
Their extensive network of aged care homes and providers covers the whole State and can match the care needs, budget and preferences you outline.
