Continually at the forefront of aged care innovation, Royal Freemasons pioneered community care in the early 1990s, and since 2001, the organisation has become a major provider of care packages across metropolitan Melbourne.
The care and support delivered through our various Outreach programs, gives eligible clients the opportunity to continue to enjoy the comfort and independence of living in their own home.
Coordinated programs are delivered by trained staff from Royal Freemasons, rather than agency staff that many other providers rely on, so it's always someone you know who comes to the door.
Our programs are individually tailored to each client's need depending on their circumstances & preferences, and include help and assistance with a wide range of personal and household tasks as well as on-going management of care.
The Home Care Packages Level 1-4 cover clients who would be eligible for permanent residential care, but who choose to remain at home.
To help clients be discharged early from a stay in hospital, Royal Freemasons operate a Transition Care Program that is available both to clients within a residential setting and those who choose to return to their home.
The community Transition Care Program provides restorative services designed to enable clients to continue their recovery out of the hospital and arranges the appropriate long term care, should it be required. An example of services includes home nursing, domestic home help, case management and transport support.
