Nutrition Care Services

Nutrition Care Services is an NDIS registered service provider for dietitian consultancy in the home. 

Participants can be referred for malnutrition, unintentional weight loss, weight management (overweight/obesity), enteral feeding, wound healing/pressure sores, diabetes management, texture modified diets/ thickened fluids for dysphagia, other chronic conditions such as gastrointestinal and metabolic disorders or any other changes in dietary habits. 

Our dietitians can assist to improve health outcomes by assessing the nutritional adequacy of the participant's diet, determine their nutritional needs based on their medical conditions, conduct malnutrition assessments, provide advice/ meal plans to clients and/or carers, educate about meal preparation, food safety and storage, arrange nutritional supplement registrations, liaise with other health professionals involved in their care and provide regular follow up to promote compliance and monitor progress.
