Supporting and maintaining quality of life and independence for seniors and their carers, living at home, is the best option that any aged care provider can offer. The Mayflower Group works in partnership with clients to achieve this aim through our Home Care Program.
The Mayflower Group currently has 30 Level 2 Home Care Packages which are managed from our Brighton site.
What is a Home Care Package?
A home care package provides a coordinated package of services tailored to meet the specific needs of the client. A skilled and experienced Case Manager oversees each individual package and is the conduit for communication between the client and the point of service delivery.
What types of services are provided through our Home Care Package?
The services available include assistance with:
- personal care including bathing, dressing and mobility
- meal preparation
- home help such as laundry, ironing, cleaning, gardening and simple maintenance
- transport to and from appointments, social outings and shopping
- clinical care such as nursing and physiotherapy.
What additional supports are offered?
We can also support clients at home with general companionship and enabling clients to remain in touch with their social networks.
We tailor our services to meet individual client needs and goals, but we are also there to support those at home caring for the clients.
In addition, those clients within our Home Care Program who have required transition into residential aged care have been assisted in this process with minimal disruption and stress through our support.
