Overview
Through the "ON My Terms" Home Care Package Program, your supports requirements will be developed with you and our skilled Case management Staff. Services may include home care, home or social respite, medications, clinical service, personal care, purchase of aids, property maintenance or modifications, transport, purchase fire wood, some electrical good and other. Our community Care Worker are both qualified and highly experienced both in care in the home and community. The service is reliable, on time and consistence with excellent work practices.

Home Care Package Level 3 - to support people with intermediate needs
