How to get started
You can access services through a Home And Community Care (HACC) Assessment. The Community Nursing Service is a designated HACC Assessment Service within the city of Moreland. If you live in the Moreland area, all you need is a referral by a doctor, hospital, informal carer, or even yourself. An assessment visit to your home generally occurs within a week of referral. Nursing and personal care staff can also provide services to eligible Department of veterans affairs.
The service is non-denominational and open to all members of the community.
Need a translator? We will organise an interpreter to assist clients from non-English speaking backgrounds whenever they are needed,
This service receives Commonwealth and Victorian Government funding under the HACC program.
