Our Home Care Services support older people to be as independent as possible in their own homes. We can provide short or long term care, transitional care or one-off support for people who wish to stay in their own home and need some assistance to stay independent and in control.
Services include house cleaning, laundry, changing bed linen, ironing, putting out rubbish bins, shopping, meal preparation, accompanied shopping, friendly visiting, undertaking social activities and outings, assisted participation in community life, personal care, transport services, home modifications, home maintenance and nursing services.
AnglicareSA Home Care Services include:
Home Care Packages (HCP)
These are provided under a consumer directed care (CDC) service model and support customer independence. HCPs offer flexibility, choice and control. Customers can choose to take primary responsibility for the management of their Home Care services, or they can choose to have a less active role. AnglicareSA offers package coordination support as well as a broad range of direct services.
Commonwealth Home Support Programme (CHSP)
CHSP supports customer independence at home and in the community. Services are accessible, equitable and affordable. Under this programme, AnglicareSA provides a range of basic support and care services to meet customer needs. CHSP is supported by the Australian Government Department of Health.
Privately Funded Services
AnglicareSA provides a large range of services outside of Government funded programs which offer flexibility and competitive rates.
Transitional Care Packages (TCP)
TCPs provide rehabilitation and support for older people as they leave hospital and return home. It is a 12-week rehabilitation program supporting older people as they transition from a hospital stay to life back at home. The program focuses on the restoration of independence, health and quality of life. Clients are referred by their hospital – self-referral is not available.
