Older people living at home can now readily access all the non-medical help they need
AgeSmart is a new premium service specially created to help the carers and families of older people to easily put in place the exact mix of at-home assistance they require, at just the right time, in just the right amounts.
We’re uniquely equipped to professionally coordinate the provision of any combination of non-medical support – from wills and powers of attorney to Advanced Care Plans, to top-up community care and health services.
We’re also geared to keep GPs fully updated on all facets of the various services their patients are receiving; and to help our clients minimise costs, we also ensure that access to all government-subsidised entitlements is maximised.
Overcoming the problems of coordinating at-home care
For too long, older people living at home (and their carers and families) have been left to navigate their own way through the maze of aged care services on offer. Many find themselves completely lost in the complex process of putting all the non-medical support they need in place in the correct combination, at the most appropriate time, in the most effective and cost-efficient manner.
Self-funded retirees, in particular, face difficulties with accessing support from the publicly funded aged care system. Since July 2015, they’ve been forced to pay extra fees by the subsidised community care sector; and from March 2017, they will be assigned a lower priority to access Commonwealth subsidised aged care services.
Historically, GPs and practice nurses have also had no real way to professionally assist patients with accessing and implementing the full mix of at-home care they’ve identified is required.
The new AgeSmart service has been specially set up to overcome these problems.
Today, our service provides all parties with a much needed means of benefitting from the expert provision and management of comprehensive coordinated care.

AgeSmart is able to professionally coordinate and implement any combination of the following services to comprehensively meet the specific at-home needs of each individual client, even as their personal situation alters over time.
The full spectrum of services we coordinate comprises:
Non-Health Professional Services: legal advice (wills, powers of attorney (general, financial and medical)), accounting / financial planning, aged care bed brokers, Advanced Care Planning, Aged Care Assessment Services.
Patient Support: bill payment, transport, advocacy at meetings, assistance with social integration, manicure and pedicure, carer respite, patient / family education, personal alarms.
Support for Activities of Daily Living (non-subsidised and supplement to subsidised services): post hospital care, home nursing, continence management, home help, delivered meals, home maintenance, aids and appliances, wellness promotion, Home Medicines Review, Webster packs.
Non-Subsidised Allied Health: occupational therapy, physiotherapy, speech therapy, dietetics, podiatry, psychology, exercise physiology.
Medical Care: AgeSmart even enables patients’ GPs to access up-to-date information on all non-medical health services being provided to proactively review online, or discuss with the patient or carer in person.
