Aged Care Planning was established in response to a growing need from families who struggle to cope with the stress and emotional practical issues relating to aged care placement, transition to appropriate accommodation and ageing.
In 2014 Aged Care Planning added a new Division - Education and Counselling to broaden its offerings to clients and families. Helene Hipp has joined the team, and brings her 25 years of experience as a counsellor and educator to our organisation. Helene specialises in palliative care education for personal carers & volunteers in aged care. She also provides counselling for residential aged care clients and their family on change management, coping with illness, loss & grief & end of life issues.
From 1 July 2014 a new Advance Care Directive form became legal in SA. This form allows you to write down your wishes and instructions for your future health and personal care and decide who will ensure those wishes are followed through, if you no longer have the capacity to communicate those decisions for yourself. Once this has been completed and distributed family, carers, and medical professionals will be able to provide care in line with your wishes if, in the future, you are unable to make those wishes known yourself. Having these discussions with family can sometimes be difficult. Helene works with clients and their families to discuss what they would like to have included, talk through the personal wishes and suggest ways to begin this vital conversation with family. Helene can also provide staff development on the use and completion of this form.
