ACH Group’s specialist Allied Health and nursing professionals deliver services that support older people to live good lives by optimising their health and wellbeing.
Complementing ACH Group’s focus on wellness, reablement and restoration is a range of specialised health programs that combine a mixture of health disciplines to produce meaningful health outcomes. Programs include: Getting Exercise Ready, Falls Prevention, Healthy Memory, Stroke Recovery, Managing Arthritis and Restoring Health and Wellbeing.
What services are available?
Using an individualised approach tailored to the person’s specific goals and needs, our health professional team will assist to create opportunities to enjoy activities they love and to try new ones. We can support our customers to maintain or develop links in their local community and help with self management of chronic conditions.
Exercise Physiology: individualised, goal orientated exercise programs are developed for people with existing injuries or chronic conditions (such as hypertension, heart disease or diabetes) or who are at risk of developing these conditions. Support is provided one on one or in group settings and includes education, prevention and self management.
Podiatry: assessment, diagnosis and treatment of foot and leg conditions leg conditions. This includes diabetes management, gait analysis, education, prevention and self management.
Occupational Therapy: assistance for people to live more independent lives by providing home and lifestyle assessments, advice and equipment to help with movement issues, home modifications, small aid prescription, lifestyle planning and chronic disease management.
Dietitian/Nutritionist Services: support with weight management, nutritional support and advice, and chronic disease management.
Physiotherapy: hands on therapy and exercises to help manage/ alleviate pain. This includes management, lifestyle planning, musculoskeletal rehabilitation, exercise development, mobility assessments and falls and balance education.
Nursing: support with health and lifestyle care in the home setting including wound care, medication management, chronic disease education and lifestyle planning.
Massage Therapy: hands on therapy to assist with musculoskeletal disorders and rehabilitation.
Chronic Disease Coordination: support and education to help people manage with chronic conditions and overall health.
Fitness Groups: exercise groups, including gym and individualised sessions, including chair based yoga, walking groups and Tai Chi.
Speech Pathology: assessment and treatment related to swallowing, speech and language.
Psychology: assessment and treatment for people who are experiencing depression, anxiety, sleeping problems, stress, behavioural problems or early signs of dementia.
Social Work: working with the person, their family or carer on issues such as grief and loss, advocacy and assistance with lifestyle planning, financial planning or Advance Care Directives.
