We specialise in supporting individuals with physical and psychosocial disabilities and chronic health conditions, and we are proud to be a registered provider under the National Disability Insurance Scheme (NDIS). Our NDIS services are designed to empower people to live more independently, confidently, and with greater choice and control over their lives.

Our NDIS Offerings Include:

• Specialist Support Coordination - Tailored for individuals with complex needs, including those in high-risk situations or experiencing behaviours of concern. Our specialists work closely with participants to navigate challenges and coordinate multiple services effectively.
• Support Coordination - Helping participants understand and implement their NDIS plans, connect with appropriate services, and work toward achieving their personal goals.
• Therapy Services - Including Exercise Physiology and Dietetics, supporting physical health, mobility, nutrition, and overall wellbeing.
• Recovery Coaching - Designed for individuals with psychosocial disabilities, our recovery coaches assist in connecting with essential health and mental health services—both within and outside the NDIS. They work collaboratively to develop and implement personalised recovery plans, coordinate supports, and help participants navigate the NDIS system with confidence.

As a Commonwealth Home Support Program (CHSP) service provider, we help older Australians maintain their independence at home. We also offer Home Care Package (HCP) services tailored to individual needs, along with residential aged care services.

Our comprehensive aged care offerings include Exercise Physiology, Dietetics and Occupational Therapy. These services are delivered by highly experienced allied health professionals committed to person-centred care across community and residential settings.

Our CHSP, HCP and Residential Aged Care Services Include:

• Exercise Physiology - Functional assessments, falls prevention, exercise and mobility programs, and hydrotherapy.
• Dietetics - Food, nutrition, meal planning, and dietary advice.
• Occupational Therapy - Assessments, assistance with daily activities, adaptive equipment recommendations, home modification suggestions, and chronic condition management.
• Psychology - Mental health counselling and support.
• Social Work - Counselling and wellbeing support to help navigate life’s challenges.
